Media tags – display url&title of posts associated with each image

I need some help with the media-tags plugin.

I’ve managed to cobble together something that almost works as I’d like (sorry if it’s too painful to read… I’m making it up as I go). The bit I can’t work out (it may not even be possible) is how to get the url and title of any posts that an image (or media item) may be associated with (in my example an image is found in post ‘Summer holiday’ aswell as ‘Best photos of 2012’. I want to display a bit of text beneath each image that says ‘this image can be found in post … and …’

Any ideas?
