Hello inflection, I replicated your error. Of course a screenshot of a blank page will not work, I was referring where you click, but don’t worry, I have replicated the error.
Please verify that the URL you are using on the settings follows your permalink structure.
The permalink for your management page (something like /manage-subscriptions or /?page_id=345). This page does not actually exist in the system, but its link must follow your permalink structure.

Like I said, you have to follow your permalink structure, so acording to what you share this must be on you manage page:
So you said permalinks are in /%category%/%postname%/
Therefore you must have the same in
Management URL: /manage-subscriptions/manage <– Try this
Management URL: /%category%/%postname%/ <– Same as permalinks.
I have the same error as you when I replicate it and this is the fix.
I tried http://www.inflection.nl/%category%/%postname%/ and http://www.inflection.nl/%category%/%postname%/manage-subscriptions before, and now I’ve tried /manage-subscriptions/manage as well but everything I try gives either a 404 or a blank page.
On the page of the article the link literally says /%category%/%postname%/ and it’s not filled with the actual category and postname.

Not for me, not related to the slash at the end, only difference that made between a blank page and a 404 page…

No, still a blank page. Adding a slash at the end or not makes no difference.
Same issue here. The plugin has been working flawless before, but out of the sudden (I guess after updating wp or another plugin) the subscription management page doesn’t work anymore and leads to 404 page. Nothing in the permalink structure has been changed.
Guys, I will dig deeper to see what is this issue. I will be updating the plugin in order to work on the latest version of WP. In the mean time you can fill the form on this page http://behstant.com/negocio/contact.php with you info so that we can have a close contact an close this issue.

The problem was indeed that the “virtual management page” link wasn’t following the permalink structure. So that’s solved
