   Hi there,

   I put the MailChimp subscription button on this website:

   http://alexisdelpalazzoflute.webs.com/ (you can see it if you scroll down the homepage)

   That is the "super slim form".

   But once the people opt-in I don't get their name of website and that is what I need.

   How can I add to the code the option that they can write their name and website?

   Use the Classic form instead of the super slim form
