Lots of problems not sure where to start

I loaded in the plugin and put two entries and it displayed with no problem so I went ahead and loaded 54 more. Now nothing works.

Here they are:
1) here is what the general settings tab says now: Error 404 – Page not found!

2) manage locations tab now shows this instead of the entries: Warning: Division by zero in /home/kemannel/public_html/wp-content/plugins/store-locator-le/include/storelocatorplus-adminui_class.php on line 596
