Looking To Make Custom Forms Without plugins (1 post)

I run a WordPress website, and would like to use that lightbox type deal when someone clicks on something a box overlays the website.

Cause, I'd like to make a custom form for people, so when they click on "REQUESTS" a form will appear over the blog itself, lightening the background of the website, and they can enter information into that certain form.

But, I'd like that information to be posted on a seperate part of the blog. Not in the "PAGES" portion of the website. If anyone knows what Im talking about or runs a wordpress blog, then you will understand.

So, pretty much they click on the "request" tab on the blog a box pops up with a form in it. they click on submit, and they can see there requests on another part of the blog maybe like youarethescene.net/requests
cause, this is going to house "2,691" members, and I'd like the opinion of going in on that request pages and deleting requests that have been filled by other users.
