Looking for custom post form that uses check boxes and text fields

I would like to have a plugin that allows posts to be created by filling out a custom form. Similar to a contact form but instead of an email being created, a post is created.

Is their anything out there that I can download and install?
I would like to mainly use this to create a custom form that allows consistant formatting of each post since it will be used primarily for adding a standard product review.
