   I'm looking for a plug in that basically says "Did you find what you were looking for?
   Yes
   No
   If not, please tell us what you were looking for"

   It needs a yes/no polling feature and the ability to add text and pass the whole thing along to the admin - count the yes and no votes and pass along the 'no' text.

   Has anyone seen a plug-in that will do that? I've run out of ideas for keywords to search!
