Well, no there are no issues out of the box, you are describing something out of the ordinary. You certainly have a conflict of some sort going on there. I have been using the plugin on 3.8.1 since release.
Deactivate the plugin and try and switch between the two existing readability settings.
You’ll have to have a look closely at what other plugins you have active and see if there is something else going on. (If you have plugins I am unfamiliar with, you may have to resort to your own ingenuity though).
Activate this plugin and try one of the additional settings, but specify which one you try. “private” is all you say, along with “every time” and “do something” so there is not much I can do to reproduce the problem. One of the settings is “registered users I add”, so it should kick you back to login if not logged in, or back to home if not a member of the blog you are trying to see. Or “admins only” and every member other than admin gets kicked, too.
So I offer this, try this plugin on an install on your server with absolutely no other plugins and see if it works. If it works, well, add plugins till you reproduce the error.
If this plugin bjorks up an otherwise virgin install of WP multisite, then yes something is amiss – just not with the plugin I figure.

I have created 3 test blogs to test each setting from your plugin and the same problem occurs with each. The blog is fine until the moment I click one of the options, from there on we are prevented from switching back as clicking save logs the user out. I havn’t tried to click on every single option on the dashboard, but certainly clicking on pages, posts, comments, media library logs you out and takes you to the login page. The message it displays is “Session expired. Please log in again. You will not move away from this page.”When you log back in again it returns you to the page you were trying to visit. Blogs not using any of the additional options continue to work fine.
We were previously using an older version of the plugin which has worked fine in the past. After updating to the latest version of WP and having to update other plugins too, when the problem arose we thought your plugin just required updating to the latest version too and expected the problem would be solved.
Unfortunaly we run a very large network with several thousand blogs and lots of plugins, so it is difficult to deactivate until we find the culprit of the conflict. I do however suspect that the problem could be related to the multi domain plugin or domain mapping plugin by WPMU Dev as the problems here seem to concur with when changes were made regarding these plugins.
Are you familiar with these plugins at all?

Creating test blogs with the same combination of active plugins proves what?
You have to disable plugins and try them in isolation to isolate the problem.
Set up your own test server and test your own plugin combinations.
No, the plugins you have are not on any of my installs. But by their titles it sounds like you have more than one that could be bjorking up your cookies.

You could try the WPMUdev Multisite Privacy plugin, odds would be better that it would be compatable with a WPMU dev domain mapping plugin.
