You seem to be using Feedburner to manage subscriptions on your site; the form you’ve added to your sidebar is provided by Feedburner.
If you want to use Jetpack Subscriptions instead, you can use the Blog Subscriptions widget, as explained here:

I noticed that you use an old version of the plugin; could you try to update to Jetpack 2.2.5, and let me know if it helps?
