Is this possible? User Questionnaire with generated summary

I’d like to add a questionnaire to my blog site. The reader would go through a bunch of questions and then have a summary of his/her choices that the reader could then send to self.

I imagine that it IS possible but needs the right kind of code – what would it take? What kind of skills should I be looking for?
