Have you tried:
– switching to the Twenty Ten theme to rule out any theme-specific problems?
– resetting the plugins folder by FTP or PhpMyAdmin? Sometimes, an apparently inactive plugin can still cause problems.
– resetting your custom permalinks back to the default setting? If this works, then review Using_Permalinks before setting a custom permalink structure back up again.

what should be the code on custom structure permalinks ?
it gives me an error when i insert the code /%category%/%postname%/

Try replacing wp-admin folder from fresh download.

that error appears, plus i cant insert image too
What server are you using is it a window server or a linux server

window server, i try to upload wp-admin didnt work
That is why you having this problems try and see if you can switch to linux server. It is known that windows server does not have .htaccess files that is needed to use permalinks.

but i already have a permalinks already /%postname%/ then i change to default , then when i returning it back to /%postname%/ i got that error
