This error happens ONLY when I'm in visual edit mode.
I'll highlight the text I want to create a link for and then click on the "Insert/edit link" button. The background becomes gray and a dialogue box pops up but isn't accessible along with everything else. Also, everything is still dark gray including the "insert/edit link" dialogue box.
Any suggestions?
Deactivate all plugins and see if that resolves the problem.

I have the same problem the button is grey, however i can hit the alt+shit+a hotkey to get the java popup to at least attempt to work however when i hit insert on the popup it just clears the insert/edit link form not adding anything to the post.
I have tried multiple versions of IE and firefox, a vista as well as xp machine... disabled all plugins still the same problem
