    After the upgrade to 3.5, we’re no longer able to insert media into a post. We can upload media, in this case photos, but clicking “insert into post” has no effect.

    Checked this with another site hosted elsewhere, with a different set of plugins. Able to insert there, so this might be associated with the specific environment or plugins.

    Have you tried:

    – deactivating all plugins to see if this resolves the problem. If this works, re-activate the plugins one by one until you find the problematic plugin(s).

    – switching to the Twenty Twelve theme to rule out any theme-specific problems.

    resetting the plugins folder by FTP or PhpMyAdmin. Sometimes, an apparently inactive plugin can still cause problems.

    – re-uploading all files & folders – except the wp-content folder – from a fresh download of WordPress. Make sure that you delete the old copies of files & folder before uploading the new ones.

    By testing plugins, I found the culprit. It’s the Custom Field Template plugin. I’ll notify the author.

    I have been trying to upload pictures to a post for the past 4 days…. and read everything that seems to make sense from your help page.
    I have deactivated plug ins and I am using the 2012 theme.

    When i click on Insert Media button it takes me to a page that gives a couple of options (insert from URL etc) but the page is otherwise blank.

    I am a rank and elderly amateur so would appreciate your keeping any help as simple as possible.
