I've just completed my first implementation of 3.0.1 in multisite mode. I have previously made about 12 installations of various versions over the last three years without any problems, so I am no expert, but do have some experience.
The problem I experience in this installation (same server as three other 'normal' installs which work fine) is that when you click on the insert image button on the post edit, it opens a new page and though the upload works, it just adds the image to the gallery and I have no way to insert it into the post as intended.

I should mention I tried this in chrome, firefox, and explorer...
I want to say that it's a bad install or a plugin conflict.
Delete the files and upload fresh from WordPress.org for starters.

Well, I've tried a re-install twice, and there are no plugins installed yet (just akismet and hello dolly defaults) as I discovered this issue on my first test post straight after install...
any other ideas?
Does it work on the site if it's NOT in multisite mode?

I guess so as two other installs are on the same server (godaddy hosting) and work fine. Now I have set up a few sites on the multisite network and other authors have started entering content while I work out what's wrong with the image insert so I don't want to completely remove it...
Check that same site in not multisite mode. It doesn't do anything to the insert image functions at all.

OK - backed everything up. Went back to single-site mode...
Same problem.

So it is an issue with the account on that server. Yes, I did read other installs on the same server are fine. Yes, it IS possible that setting son one web account are not the same as another. ;) The fun of dealing with webservers.
Gonna have to hit up support on your host.
