I have this error where when I click upload image, the uploaded will load below at footer. The worst part I can't even add any image.
I face the same problem when viewing theme at theme section.

Is this happening on a new install or a blog you had before which suddenly acted up with the media upload? In which browser is bad behavior showing up?
Deactivate all plugins and change to default theme. If you already are on default settings, reupload WordPress files and folders via FTP or cPanel.

This is old blog. Problem only after upgrade to latest version.
I have deactivate all plugin, the upload now I can see but its open in full screen. I managed to upload few image however when I try to insert image, the page become blank white page.
And I am on default theme now.
I will try reupload wordpress files and folders now.
