Icons not displaying anywhere

I followed the instructions on the FAQ tab to link images to categories, but they do not display anywhere (single post, category page, home page, etc.). I saw on your site (http://www.category-icons.com/2011/01/brand-new-plugin-category-icons-lite-alpha/) to make sure my template has the post title preceded by the permalink and its title. I checked index.php and single.php and neither contain that code. Is there another file I should check?

I am using the Graphene theme and running WP 3.5.1. Website is http://www.rochester.edu/it/notices.
