I've modified a plugin. Maybe modified is the wrong word; I've used large pieces of code and inspiration from another author's work.

How do I proceed? What information do I need to include about the old plugin...? Is it ok to release this one under a new name?

Consult with the plugin author you lifted code from...

Yeah, email the plugin author and see what his opinion is. Best way. Ask about what license his plugin is released under. Usually it's GPL, but you never can tell...

I've made some attempt to reach the plugin author, without success (so far).
The original plugin is released under GPL while one component used in it is Affero GPL and another is Lesser GPL.

Well, the real factor here is what you actually created and/or modified.

The LGPL component is no issue, if you modified it, you just have to release your modifications to it and it will be LGPL licensed as well.

The AGPL is identical to the GPL except that if the original plugin offers the ability to download the source code, then your plugin must have the same ability.

And the GPL is the GPL, which everybody knows, pretty much.

You can release your versions of any of these without difficulty.
