   I have Shipping set to Flat Rate with free shipping > $250 and my only market is USA (flat rate is 50 states only).

   At the top of my checkout page it shows "Please choose a country below to calculate your shipping costs" with the State pull down just below that. This is very confusing for the customer and gives a low quality appearance.

   If I turn off shipping, it goes away.

   I had this problem. The only way i got round it was to use the wspc-simple-shipping plugin, assigning the different shipping costs and then hid the 'please choose a country below...' using some css.
