How to place Custom Fields into the url sturcture of a custom post type.

I have a theme that creates local business directories.
The default url structure is http://cashforgoldbuyers.us/listing/aaa-gold-exchange-13/
or
url.com/listing/postname/
I have custom fields such as state and city (CA and Palm Desert) that I have set up. I would like to be able to use the same custom fields in the url structure of the listing.
ex: http://cashforgoldbuyers.us/ca/palm-desert/aaa-gold-exchange/.
How do I go about this?
