How to get CPT data from post_name into custom page?

    I have a custom table that contains some data that I use to populate a custom template.

    In this custom template I now want to call some data from a custom post type of Profile where the post_name = a surname pulled from this other table. Everything I have managed to find so far doesn’t explain how to do this.

    Any tips?
