   Hi,

   I want have a page on my wordpress which should look like the following.
   http://travelquila.com/test/index.html.html

   This page references some Java Script Files from the same server.
   It is a complete HTML page code. When I try to put this into the HTML section of WordPress New Page Window; only text is displayed. The Checkboxes and Submit Button are not displayed when the source code of the above page is pasted in HTML Page code of WordPress New Page Window.

   Please suggest what can be done?

   Create a custom page template for your form.
