I have setup my blog and superadmin per the instructions in the codex. I have been able to create new subdaomin sites as a test.
Now, I would like to create a registration format similar to that of WordPress.com where it is extremely easy for people to create a new site. Also, i'd like to include the top toolbar and search functions found on WordPress.com.
Is there a plugin for this or can someone point me in the right direction to accomplish this?

Go to yourdomain.com/wp-signup.php There's your signup page, almost identical to wp.com.

I have followed your instructions and installed the plugin. However it still does not give me the option to "Register a New Blog". What am I missing?
You missed the point where the plugin was to add an admin bar.
the signup page is built in.
you do not need a plugin.
Go to yourdomain.com/wp-signup.php.
What do you see?

When I go to yourdomina.com/wp-signup.php I get a page at wp-login.php?action=register that wants me to register for my existing site.
Go look at Super Admin -> Options -> Registration options.
Which one do you have selected?
Did you enable multisite at all? Because that's what adds new blogs. Not a plguin. A feature you enable.
