How to create a form for a Solve Media puzzle and a download button

How can I create a form to download a file with a Solve Media puzzle. I need users to complete the Solve Media puzzle and than click a download button to get the file. An example would be found here: http://www.livemixtapes.com/download/26285/migos-no-label-2.html

I have already downloaded the Solve Media plugin for WordPress. I would really appreciate help with completing this. So far I can only get the Solve Media puzzle on my page but there is no button for users to submit their answer. As you can see here: http://www.ilistenloud.com/solve-media-test/
