   I have not used the User Photo Plugin, and it looks like it's not being actively maintained at this time. I might add though that there are users who've reported that the plugin's working in version 2.9.1 on the plugin page.

   If at this point, you haven't gotten the plugin working yet, might I recommend Peter Sterling's Add Local Avatars Plugin where you have option of using your gravatar or locally uploaded avatar. Works in all my WP and WPMU installations.
