How to capture user input

Hi

Does anyone know if there is a plugin out there that lets external users enter tick-box style information for a predefined set of fields?

Concretely I would like to capture soccer practice attendence for a team I am coaching directly from a page and be able to store and report back statistics by team member.

So would need to enter date, and predefine each team player with a tickbox facility for each of them indicating if they were present or not.

I have looked at poll-style plugins, but those I have found doesn’t quite do it for me.
