1: i can’t able to create more than 3 access level
This is because this free lite version of Membership plugin only supports maximum creation of 3 access levels. If you want to create more than three access levels then you should use our full Membership plugin.
2: Content is visible to all…
If you don’t want to display it to all then set Use membership level to “None – No access to content” in the plugin stranger settings from the following path.
Admin area -> Membership -> Options -> General Tab

You should add the rule in a Positive Rules section of the access level.

It seems from the shared screenshot that you have configured it correctly and it will display only the posts attached to “Europe” category to the users having access level as “Mid” and for all other posts the Protected content page will be displayed.
Have you added any other rules apart from “Categories” to “Mid” access level?
You can check how the site content will be displayed for specific access level by selecting it from the drop down “View site as:” displayed in the admin bar. ( See the screenshot : http://i.imgur.com/fMwzmiG.png )

no other rules set to mid level. i set one user as mid level when x user login with own userid password he can access all post and categories 🙁
if you want i can share backend details with you…its important for me
its working when i see view as X level ..but when login with as user nothing showing everything is blank
