How do I show new forms within a custom page template? (3 posts)

I'm new to php and have spent hours trying to figure out how to fill out a login form created in the custom page template php file, submit the data, and replace the form with a menu. Include just adds content to what's already there and the header(location) function takes me to another php file but then I lose the page template. This is probably so simple, but I'm totally confused. Any help would be appreciated. Thanks!

Have you looked at plug-in to do that?

Sorry, I should have been clearer. The login form is to gain access to some custom tables I created within the WordPress database. Logging in isn't the problem, it's changing what is displayed inside the custom page template.
