All I want to do is move my blog off my domain to http://www.mydomain/blog.
I have read:

The point I don't understand is how to create a new directory.
I have also read:
http://codex.wordpress.org/Giving_WordPress_Its_Own_Directory
and I'm still none the wiser.

a directory is a folder.
you use ftp, or a file manager if your host offers it to create the new folder...which in your case would be named blog
Its like making a new folder on your computer...same thing. a folder is a directory, and the stuff in it is the files
