How can I collect data through a form?

I need a way to get data inputted by a user through a form (email, age, sex, state, city) organized in the dashboard. Kind of like a newsletter, only I don’t need to actually send anything out. I just need to collect the info and maybe have a way to export the data.

So, for example, if someone fills out the form and hits ‘submit’, the data gets arranged neatly in a section of the dashboard. Is something like this possible?
