headers and bottons in directories folders

I have created directories and changed info on ebooks w/resell rights w/no problems. The pic files always stayed with them and everything uploaded fine. When I create my own I cant get the headers, buttons or any graphics to come up on the documents. What is the trick to this?
Do I need to put an execute file in the page to look in a folder for a picture? Anybody know of a good resource to find out how to do this?
