OK, well you have a couple of choices…the plugin doesn’t have this kind of functionality at all, so you’ll either need to just let them use a text area or actually do some coding and create a custom form element that can serve as a daily log.
You may actually be better off registering your users on your site, then using access control to give them access to their logs (using some kind of plugin to track their daily diet). A little complicated to set up, but then what you want to do is pretty complicated unless you have a plugin that is designed for this purpose.

Thanks. I am seeing this in your FAQs:”An individual record can be edited on the frontend of your website by someone who has the private link to that record. The purpose here is to let people maintain their own records. It’s also possible to put things like survey questions in the form so you can get feedback from people. You have complete control over what fields appear on this form. Fields can be designated as showing up on the signup form, on the frontend user’s form, and on the backend admin form.”
and wondering then if the information from this form can be used with your plugin instead of the Google Form:

and then a summary:
http://www.bodybyginny.com/challenge-scores/
Well, maybe, but you’re not going to avoid needing to develop custom code to get it to work because you’ll still need to link the two together somehow.
Your biggest problem is going to be the fact that the Participants Database doesn’t have any kind of login or a way to know who a user is. The private link only lets the shortcode know which record to show for editing, any other plugins on that page will be visible to anyone. This is why I suggest going with a plugin that gives you registered users.
