I've hardcoded this simple form into a page template, but when I hit submit it redirects me to a 404. I tried to remove the action value, but it still doesn't work. Any suggestions? Here's the page.

Problem solved. I had to prefix the form fields, as suggested by Kawauso on #wordpress (freenode).
