   Was just wondering if anyone could help me with adding a sidebar widget for the Google Maps Store Locator for WordPress http://www.viadat.com/store-locator/

   Basically what i need is for the address search field and radius limit select box to be in a permanent position in my sidebar, Then when the address is entered the user is taken to a new page which will display the map and the store locations.

   Any help would be greatly appreciated.
