get plugin running JQuery Html5 File Upload

    Can you be more specific with how to get this plugin running on a wordpress site?

    I have uploaded the plugin and activated it. However, I don’t know how to get the upload form to show up on a page. Usually with a plugin you have to put a tag on the page for the plugin to display. If you can please provide steps and instructions it would really help.
