    Not to sure if this a bug or not, but generate_rewrite_rules appears not to trigger if ‘default’ permalinks is selected (appears to fire for all other options in permalinks).

    Is this expected behaviour? If it is, what is the best way to go about adding custom rewrite rule for use in a plugin?

    I may be wrong, but if the default option for permalinks is selected then no rewriting happens, so any custom rewrite rules won’t be used anyway will they?

    Yes, I believe the reasoning is the Rewrite API is to extend the permalink functionality. If the user should choose to not use permalinks at all (“default permalink” is an oxymoron), it stands to reason the Rewrite API should not be applied either. It may not be reasonable from your point of view, but there is logic to it, even if you disagree with it.
