Discovered today while looking at validation a possible hack that is going overlooked by people.
If you check your posts for the following code;

You may have been hacked. I have googled it and I see it on hundreds of sites but no one is discussing its source. I've found it in blogs, forums, joomla sites and today in a ticket system.
