Form with Multiple Text Areas and other features

    I am looking for a form where I can add text areas, droppdowns, checkboxes etc., possible multiples of each. I want so set up a for with many areas to collect details.
    Is there such a form?
    I have installed “Secure and Accessible PHP Contact Form v.2.0WP” but it doesn’t seem to do as much as I’d like.
