   Hello,

   I need to add form on the top of button "add to cart" ?

   I try with "iphorm uniform builder" plugin but in this case i get two buttons (1. for form submit and 2. for add to cart)..

   Try looking at gravity form product addon plugin...It is in the woocommerce extensions area of woothemes.com
