[resolved] Form created but no lists showing (6 posts)

    I have created a form and have 4 lists on Constant Contact. When I add the shortcode on my page, the fields I chose (first name, last name, submit) are visible but the lists are not.
    Any suggestions?

    Thanks Zack. The code works, except all of the lists show, even if on the registration and profile tab I only chose 2 lists.

    Zack, I have a website that has no login requirements. I am trying to get the plugin into my front page footer. Input form is there but no lists. What do I need to do and where. Please don't link to the pages above. I have read them in depth. If I have to put some code somewhere, I need to know in what file. If I need to put a shortcode, I need to know where since I am using it in a widget. Thanks in advance.
