Fix some small canonical redirect bug (1 post)

I've been trying for some time now to force redirects to occur in this specific instance, but with little progress.

When a user visits a blog article page at the url http://www.siteurl.com/%category%/%postname%/ we want it to redirect to http://www.siteurl.com/%category%/%postname%/%post_id%, which is how our permalink structure is configured.

I've tried hooking to 'send_headers' and just forcing a redirect to the permalink, but I can't seem to get the permalink from this hook.

Does anyone have any suggestions on how to handle this?
