i want to use file uploader facility in only some of the pages. i am getting this as a widget but i don't want that. widget is visible in all the pages, as it is placed in sidebars. Is there any plugin available that allows me to use only in some particular pages. Thanks in advance.

It's a form maker, you put it on any page you want
It can be configured to allow file uploads, I use it on my site for that purpose
