I'd like a file upload form on my blog so users can upload pictures and after I approve, the uploaded pictures show underneath the form. Is there a plugin that can do this?

You could manage your photos with NextGen Gallery
http://wordpress.org/extend/plugins/nextgen-gallery/and then add the NextGen Uploader so users could upload images
