Feedback form mysteriously appearing

I have a contact form mysteriously appearing on my contact page. Since it was however not connecting to my email address and sending me notifications, I went to edit or delete the form by logging in at the backend of my site and searching for the code.

This code or form is nowhere to be found in the text or visuals where I edit the pages. I have no way of deleting it or editing it. This does not appear on any of my other pages.

I have looked to see if I could find it elsewhere to no avail.

I even deleted the contact page and started again, but the same thing happened, the form appeared.

As a result I now have 2 contact forms on there- the jet pack one that does work that I installed, and the mystery one that won’t go away and doesn’t work. (Even though it says email has been sent, it doesn’t)

I’ve been missing new students and work because of this form not working, so any help is appreciated.
