I just updated to svn trunk and the 3 dropdowns: Email Settings and Notif bar settings are just empty. You click the Down arrow, and there are 0 options in the selection dropdown. Additionally, since I see you went the dropdown route, there is probably no way to specify an email directly, unless you create a user and assign an email to that user.
Could you uninstall plugin from dashboard->plugins and then add and install again?
I couldn’t get it to uninstall because in order to do that, FTP has to be set up in WP. I just hunted down the settings in the database and wiped those instead. All working now.

Now that the dropdown is working, I can see how you implemented custom emails. Works great.
Can I reopen this to suggest the ability to specify custom emails AND the post/page author? Right now that’s not possible – the only option if you want the author is admin, but not custom emails.
Use case: post author and 3 editors need to be alerted.

OK, I think there’s a bug. If you report from the post page, it emails the author too, along with the custom emails. But if you report from the list page with multiple posts, it only sends to the custom emails, not the post author.
