evernote intregation

I want people to fill out an online form and submit a picture. After they submit, I will look at the info in my evernote. For example – I will have many folders – some of them will be called Jewish Guys – New York; Jewish Girls – New York, Jewish Guys – Los Angeles, etc. You seem I’m trying to get people to fill in a profile and have it entered into a working system like ever not so I can review them all easily. how can i do it like this?
