Error when posting comments

HI All

I have a problem when people post to my website and I can’t find any reference to it elsewhere. When people post, they get a blank page with code like this at the top:

The post does actually work, but the user doesn’t know that it has. I’ve looked everywhere for a solutions, but I can’t find any other reference to it, only to 404 errors which are not happening. Please help!
