Error processing the form code

    I followed all the steps and have my Automatic Sign-up Form code in HTML – I have copied and pasted this into the widget – but keeps saying: The code below didn’t work.
    There was an error processing the form code you entered into the “Automatic Sign-up Form Code” field.

    I have the latest version of wordpress and I am note sure why this is not working.
