Hi.
A few questions about the excellent Contact Form plug-in.
1. Is it possible for a user to click a radio button and then have a drop-down menu appear after the button is clicked? This would be an incredibly valuable feature.
2. Is it possible for there to be two levels of choices in a drop-down menu? For instance, if someone was choosing a date for a class, the first option would be Weekends, and then there would be a list under that of different options, and then later down the list it would say Thursdays, and then more options. Both Weekends and Thursdays would not be able to be selected.

One more thing:
3. The contact form's input boxes appear above other drop-down menus on the site. Is there a way for them to sit behind everything else on the page?

I am having the same issue with out much luck fixing it.
Using http://wordpress.org/extend/plugins/contact-form-7/
and http://wordpress.org/extend/plugins/multi-level-navigation-plugin/
Would love a fix for this, also if I find one I will come back and post.

This fixed it for firefox on a mac, not tested in IE...
Add z-index: 1000; to #suckerfishnav ul {
