Download/virtual orders must be approved despite settings

I have the option “Grant access to downloadable products after payment” checked off and all items are virtual and downloadable yet the orders must be manually marked as completed for people to receive their download links email message. I’ve read the online docs and everything seems to be ok. I’m using PayPal. Any ideas?
