If the plugin is ran on a site to change thumbnails back to 150x150px, what happens to the thumbnail images that were embedded into a page or post? Do they automatically switch to the new 150x150px default after?

Yes. Example:
We have a entry add_size “thumb1” for 150×150.
If we add some other or change it, all the old thumbnail will be deleted and created again with new format.
